That error usually indicates the patient study list is corrupted. That database is stored in 3 different directories on 2 of the hard drives. However, reinstalling software should have overwritten any patient studies in the directories. If you haven't done so, try resetting the patient database. In Setups, go to Print/Network > Global Config. Click on HD Reset or Reset Patient Database (depending on your software version).

Rats! If you were able to reinstall software with no problem, then it sounds like the Host motherboard or the DSC is bad. Try reseating the RAM on both motherboards (assuming you have an A, B or C cart). Also, try reseating the DSC in the Host.
I hope you are lucky and it was a software corruption that has been cleared up. Philips improved the Application software over time so that it could repair more software glitches. However, it may be an intermittent and that would be consistent with connection problems on the Host. As we say here, let a sleeping dog lie, which means, if it keeps working, don't touch it. I suggest making back-ups of the system presets and of the hard drives while everything is working correctly.
You may know this already, but I want to caution you about working on these older back ends with dual motherboards, especially the Rev A version of the iBoxes. If or when you decide you need to reseat the boards and components, you should remove the Host assembly being very careful to mark every cable since it is critical that they are reconnected to exactly the same outputs on the Host (especially the USB and firewire cables). As you may already know, be careful of the metal edges on the sheet metal. Placing the Host assembly on a table with ESD protection will make it easier and safer to remove and reinsert the RAM. Also check the Xeon processors on the motherboard; there were manufacturing defects on rev A Hosts where there wasn't a proper application of heat sink compound. This will cause the processor to heat up and causes slow operation and intermittent lock-ups and errors.
