An educated guess. You might check your hand, foot, and control panel exposure switches. You can unplug the foot and hand switches to eliminate them but you will have to ring out the front panel switch. Most x-ray systems don't like to power up with an exposure command initiated.
According to my service manual that error is a generator operation error code. It means KV was sensed without X-Ray requested. This indicates a fault with the high voltage generator.

I have an OEC 9400 c-arm with "KV ON IN ERROR" when it tries to boot up. Does anyone recognize this error code?
