I am facing following problems with the QDR 4500 having version 12.5, the problems' frequency is increasing day by day:
1) Scan ending due to error condition see details for nature of error and possibility for recommended action. If no action is recommended, contact to Hologic customer support.
Details: X-ray firing order is out of sequence. State machine error in task 0 index 349.
2) Unexpected error 5001004 in routine x-ray get line frequency ( ).
Details : Driver not installed.
3) Scan ending due to error condition see details for nature of error and possibility for recommended action. If no action is recommended, contact to Hologic customer support.
Details: Unable to position derive within specified tolerance. State machine error in task 0 index 258.
Could anyone help me to diagnose the problems related to?
you probley need to change the filter drum belt and realign the filter drum assembly. The belt will skip over time as it wears down and will cause these errors . you can buy from hologic as a kit assembly to replace

Exit, repeat the QC procedure and check the phantom position carefully. System
failed. Check that the phantom is the ONLY object on the table.
Test failed - mean exceeds limits. An error has occurred in a CSFStream."
Would you decode this error and help me out to resolve this issue.
The firing order is determined in the Filter Drum. Index Errors originate in the filter drum and motors. Belts and alignment of the filter drum may solve the problem. There are times when the IR chips on the back of the Filter Drum cause Firing Order failures. Failures in the C-Arm Interface PCB or ComCon PCB are rare.

I THINK YOU HAVE TO REPLACE THE X-RAY TUBE BUT FIRST TRY TO REPLACE THE CONTROL BOARD...
