Between 2001 and 2013, spending on skilled nursing, long-term rehabilitation facilities, and home health services for Medicare beneficiaries rose from $27.5 to $53.8 billion, a rate of growth that outpaced overall health spending. The Centers for Medicare and Medicaid Services have turned to a variety of value-based initiatives to slow the rate of growth in post-acute care spend and reduce the number of hospital readmissions.
