The Practicum is a distinguishing feature of DHLPMR. Resident activities during the practicum include:
* Completion of a comprehensive patient care improvement project that focuses on enhancing the technical, service, and cost excellence of care for individual patients.
* Use of disease registries and patient care databases to inform the improvement of patient care using data at both the individual patient level, as well as at the group or cohort level.
* Application of systems-based patient care including needs assessment, process analysis, project planning and implementation, and evaluation of change
* Development of a final report that clearly and scientifically documents the improvement of patient care
* Close, ongoing, individualized coaching and mentoring around practicum design, development, execution, and review
* Continued integration with home clinical residencies to maintain knowledge and skills
The Ideal Graduate
It is envisioned that graduates of this program will be able to:
* Lead the change of health care systems to improve the technical, service, and cost excellence of patient care
* Link the care for individuals with the improvement of the health of populations and communities
* Lead learning in the general competencies of the ACGME and ABMS - patient care, medical knowledge, practice-based learning and improvement, interpersonal and communication skills, professionalism, and systems-based practice.
