Solutions for Connecting Care through Continuous Health

In its booth (#1725), Philips will spotlight its full suite of telehealth programs, including critical, acute and ambulatory care programs. As part of these demonstrations, Philips will showcase its applications for at-home patient monitoring, including the newly released eCareCoordinator 1.4 application which extends its population health management capabilities. With the addition of LanguageLync, the FDA 510(k) cleared application now supports delivery of surveys in 32 languages to patients using eCareCompanion, enabling care teams to work more closely with 99.9% of the US population. The update also adds behavioral science-based educational videos to support at-home care for heart failure and diabetes patients, as well as readmissions reporting that enables teams to easily mine outcomes data to gain insights around patient outcomes and identify coaching opportunities to reduce readmissions.

The booth will also highlight:

At-home patient monitoring apps including the eCareCompanion for remote patient monitoring and DreamMapper, a patient engagement tool for sleep apnea patients that has shown a 22% increase in adherence to therapy[2];
eCareCoordinator population triage dashboard;
CareSage, a predictive analytics solution to help identify patients at risk for ER transport in any upcoming 30 days;
Medical alert services;
Medication management solutions;
Personal Health Solutions, including the Philips health watch, Bluetooth-enabled blood pressure monitors, connected body analysis scale, and ear thermometer;
Two Philips customers will discuss their successful applications of Philips telehealth programs: Advanced ICU Care in St. Louis, MO, and Banner Health in Phoenix, AZ.
