PLANO, Texas – March 23, 2015 – ZeOmega Inc., the market leader in population health management (PHM) solutions, today announced the acquisition of HealthUnity® Corporation, an innovative provider of interoperability solutions that enable providers, payers and public health organizations to streamline the secure exchange of patient data across disparate systems.

HealthUnity offerings, which include private and public HIE, master person index, referral management and patient consent, have already been deployed at several integrated delivery networks (IDNs), health systems, group practices and state collaboratives. ZeOmega’s Jiva is the industry’s most comprehensive population health management platform, seamlessly delivering analytics, workflow, evidence-based content and patient engagement capabilities required for organizations seeking to transition to value-based care. Combining the companies’ capabilities will create a powerful, rapidly deployable PHM infrastructure that payers and providers alike can use to drive value-based care, not only within their organization, but also within their communities.

“Lack of EHR interoperability is the biggest barrier faced not only by providers seeking to deploy PHM and care coordination solutions, but also by payers seeking to collaborate with providers in their network. Our vision is to provide a low cost, rapidly deployable, population health management infrastructure that solves this interoperability problem, enabling organizations to leverage Jiva to empower the physician by delivering actionable intelligence at the point of care,” said ZeOmega Founder and CEO Sathya “Sam” Rangaswamy. “HealthUnity’s HIE offering, which has proven integrations with more than 35 of the industry’s leading EHRs in both ambulatory and acute care settings, is core to this vision.”

The combined company is uniquely positioned to help providers and payers share clinical and claims analytics with the goal of improving outcomes while reducing the cost of healthcare in the evolving value-based care environment.

“The ability to deliver not just gaps in care, but also care plans and patient-specific HEDIS, Star or other measures from multiple sources into an EHR is the paradigm shift that the industry has been looking for to advance value-based care without impacting physician efficiency,” said Nandini Rangaswamy, ZeOmega’s executive vice president and chief strategy officer. “The ability to do this quickly and among different providers and payers uniquely positions ZeOmega to become the value-based care enabler for healthcare organizations.”
