PinnacleHealth, a 750-bed, non-profit hospital and healthcare system serving Central Pennsylvania; and CentraState Healthcare System, a 272-bed, non-profit medical facility in Freehold, NJ will serve as beta sites for Soarian(R) Plan of Care, a solution designed to support inter-disciplinary care teams in defining and helping to manage patient problems, interventions and expected outcomes, to provide care in order to achieve desired outcomes and to generate new knowledge at the point of care.
Soarian Plan of Care, which was demonstrated this week at the Healthcare Information and Management Systems Society (HIMSS) 2008 conference in Orlando, provides nurses and other clinicians with a dynamic solution that can assist in enhancing workflow, streamlining communication among the care team, supporting evidence-based patient care decisions and delivery, and increasing compliance.
Workflow will be enhanced through Soarian Plan of Care in many ways. For example, clinicians will be able to quickly and easily access details on patient problems, activities and goals, helping to improve communication among care team members and to reduce time spent locating crucial patient information. As a result, clinicians who are beginning their shifts will be able to quickly and efficiently access each of their patients' updated information and determine what next steps need to be taken in each plan of care. In addition, Soarian's workflow-engineered infrastructure allows for alerts and triggers to take place anytime a patient assessment or plan of care has not been completed within a dictated period of time, reminding care givers of the necessary actions to remain on schedule. This specific function can also assist in increased compliance of many patient safety and quality initiatives, such as those developed by The Joint Commission and the Institute for Healthcare Improvement (IHI).
